髓過氧化物酶(MPO)是一種四聚體含血紅素蛋白,主要由中性粒細胞大量產生,在其中發揮重要的抗微生物作用。當粒細胞脫顆粒時,MPO會被釋放到細胞外空間。作為中性粒細胞“呼吸爆發”的一部分,它能通過過氧化氫(H?O?)和氯離子(Cl?)生成次氯酸(HOCl)。此外,MPO還能利用過氧化氫氧化酪氨酸生成酪氨酰自由基。次氯酸和酪氨酰自由基均具有細胞毒性,可有效殺滅細菌及其他病原體。遺傳性髓過氧化物酶缺乏癥會增加個體患免疫缺陷的風險。
研究表明,MPO水平升高與冠狀動脈疾病相關。2003年有研究提出,MPO可能作為胸痛患者心肌梗死的敏感預測指標。此后,大量文獻(已發表逾百篇)進一步證實了MPO檢測在心臟病患者中的臨床價值。2010年的補充研究進一步優化了這一臨床應用:通過聯合檢測MPO和C反應蛋白(CRP),相較于單獨檢測CRP,可更精準地預測死亡風險。
